Quality Initiatives

At St. Cloud Medical Group, our Quality Initiatives focus on preventive care, and for patients with pre-existing conditions, management of those conditions. The Quality Initiatives provide clear directives and goals for patients, and a framework for continuous, well-managed care.

Diabetes

St. Cloud Medical Group providers and the American Diabetes Association
recommend patients with diabetes meet the following goals:

  • Hemoglobin A1c tested every 6 months and results less than 8
  • LDL (bad cholesterol) tested every 12 months and results less than 100
  • Blood pressure less than 140/90
  • For patients between 41-75 years, who also have cardiovascular disease, take an aspirin every day (unless directed otherwise by your doctor)
  • Do not smoke
  • Obtain a yearly eye exam
  • Obtain a yearly foot exam

Cancer Screening for Men

While there may not be a fail-proof way to prevent cancer, appropriate and timely screening for cancer makes the most of treatment and recovery.

Talk to your provider yearly after 50 for PSA blood screening and physical exam.

Women have health concerns that are unique, and need different tests and screenings for the early detection of cancer.

Child and Teen Well Checks

Regular child and teen well checks are designed to review a child's physical and developmental status, as well as ensure overall health. These appointments provide an opportunity for you to ask questions regarding your child, and allow the provider to address important issues effecting health such as vaccines, diet, and safety. St. Cloud Medical Group physicians and the American Academy of Pediatrics recommends well child checks according to the schedule below.

Cardiovascular Disease

St. Cloud Medical Group providers and the American Heart Association recommend patients with cardiovascular disease meet the following goals:

  • Blood pressure less than 140/90
  • LDL (bad cholesterol) tested every 12 months and results less than 100
  • Do not smoke
  • For patients between 18-75 years, take an aspirin every day (unless directed otherwise by your provider)

Asthma

If you or your child has asthma, your provider will provide you with a Patient Asthma Action Plan on a yearly basis. Patients with asthma should see their provider twice a year and undergo a lung function test yearly.

 

Measuring Quality

Minnesota Community Measurement is a non-profit corporation whose mission is to improve the quality of care in Minnesota. It does this by measuring and reporting the quality of care in clinics throughout the state. You can view St. Cloud Medical Group's results and compare them with other clinics at www.mnhealthcare.org.
